You’re right. Sam wouldn’t save Dean if that was against his wishes. Remember 3x15 when they had a solution using science not dark magic that could have kept the two of them alive long enough to find something else and it only would have meant, as Sam put it, “needing a liver in half a century.”
But Dean didn’t want them to do that cause it made him “not human” (which, what do you think shoving an angel in your brother makes him?). So, did Sam hold Dean down and force that science on him? No, he absolutely did not. Even after seeing his brother die over 100 times, even after living through the loneliest six months of his life where he lost himself, he still stopped when Dean said, “Stop.”
Dean didn’t give him that courtesy. Dean didn’t even ask him if it was okay, didn’t present it to him, just had an angel go inside and use Dean’s face to manipulate Sam into a “yes” that was meant to be for fighting his death which was a thing that was actually happening, you know, coma, and instead the angel made the “yes” mean he could possess Sam, and Dean was okay with that. He’d do it again. And he let Gadreel wipe Sam’s memory too, which Sam had told Dean when Dean had Cas do it to Lisa was the worst thing he’d ever seen Dean do. So, that would be a no, if he’d asked about that. But he’d do it again.
Sam would save Dean any day of the week, except in the instances like Dean being in a coma (which hasn’t happened), Dean’s heart being too weak (remember when they went to a faith healer and Dean could have said no but he didn’t he went up there cause Sam gave him a choice even though he lied about where they were driving to), Dean having sold his soul and being on Hell’s waiting list, except any of those instances in which Dean had said, “No.”
He would never take away Dean’s choice there. That’s what Sam’s saying. Sam respects his brother’s choices more than he needs him to be alive, and you may not think he needs him to be alive, but he needs him to be alive.
